What personal information do we collect on this site, and how do we collect it?

When you participate in an online CME activity offered by ICPME, we ask you for your name, degree, affiliation, street address, telephone number, fax number, and e-mail address.
When you complete a mailing form to receive a self-study CME course, we collect the same information except for your degree.

How do we use the Information that we collect?

We use the personal information we collect to grade your posttest and to send you a certificate of completion of the CME activity. If we use a third-party company to grade your posttest and issue certificates of completion, we will give this information to that company for that purpose only.
For each CME activity that you take, you must complete an evaluation questionnaire that asks if you are willing to participate in a follow-up survey. If you state on your evaluation questionnaire that you are willing to participate in a follow-up survey, we will use your name and contact information to send you the survey. (We will share the survey results with current future faculty and with the commercial supporter of the CME activity, but the form in which we disclose the survey results to them will not reveal your identity.)
The commercial supporter of a CME activity may ask for a list of the people who participated in an activity for documentation of the first level of outcomes-based evaluation in CME. If requested, we will provide the supporter with your name, title, and affiliation, but we will request in writing that the supporter not contact you directly for any purpose.
If we sell, assign, or otherwise transfer the business of this site, we will transfer your personal information to the buyer and require the buyer to agree to treat it in accordance with this Privacy Policy.
If we are required to disclose your personal information by any applicable laws, court orders, or government regulations, we will comply with such laws, orders or regulations.

How can I be sure that my Information is secure on your Web site?

We take reasonable steps to protect your Information as you transmit it from your computer to our site and to protect your Information from loss, misuse, and unauthorized access, disclosure, alteration, or destruction.
